Summary:
Responsible for performing a case management assessment; pre-admission screening, insurance pre-certification, coordinating patient/family conferences; coordinating patient/family education and training related to the patient’s diagnosis, needs, and discharge. Involves the patient and family in developing and monitoring short and long term team goals; coordinating the patient’s rehabilitation needs; and coordinating/completing discharge planners, medical record coders, therapists, internal/external nursing staff, dietitian, psychologist, and discharge referral sources regarding the patient’s rehabilitation. Initiates and completes the IRF-PAI assessment, LOS, CMG, 60% compliance and discharge. Coordinates unit performance improvement initiatives. Promotes teamwork and cooperation within the department.
Education Requirements:
- Registered Nurse
- Valid Kansas license and appropriate education in nursing
- 1-2 years Rehabilitation experience preferred
- Current BCLS must be maintained
- Willingness to sit for CRRN exam once eligible
